Medication Reconciliation

Medication reconciliation is the process of creating a complete and accurate list of all medications a client is taking ( including key information such as drug name, dosage, frequency, and route) and comparing that list against the physician’s admission, transfer, and/or discharge orders. The primary goal of medication reconciliation is to provide correct medications to the client at all service or care delivery transition points during treatment.
